To become a Healthwise dealer, please print and complete the attached dealer application.  Once completed, please fax the application along with a copy of your state's resale certificate (required for wholesale pricing to apply) to 916-463-0176. All dealer applications are submitted to our manufacturing partners for their review as they ultimately determine authorized dealer status.
